What is a wisdom tooth and why does it cause problems?

Wisdom teeth are the rearmost molars in the mouth and usually emerge between the ages of 17 and 25. Often, there is too little space in the jaw, which can cause them to:

  • grow in at an angle
  • maar gedeeltelijk doorbreken (gedeeltelijke impactie)
  • helemaal onder het tandvlees of bot blijven liggen (impactie)

This can lead to:

  • pain and swelling in the back of the mouth
  • Inflammation of the gum (pericoronitis)
  • hard-to-clean areas → cavities and gum problems
  • pressure on the neighbouring tooth, causing shifting or damage

In these cases, removing the wisdom tooth may be the best solution to prevent further problems.


Does my wisdom tooth really need to be removed?

We do not remove wisdom teeth automatically. First, we assess whether it is really necessary. Removing a wisdom tooth is usually advisable when:

  • you experience repeated inflammation or swelling around the same tooth
  • there is a deep cavity that cannot be properly treated
  • the wisdom tooth is pushing against the neighbouring tooth and causing damage
  • an infection around the root tip is visible on the x-ray
  • your orthodontist (braces) advises removing the wisdom teeth

Together we discuss the advantages and disadvantages. You will always receive a clear explanation and a personalised treatment plan.


How does wisdom tooth removal work at 3D Tandarts?

1. Intake en onderzoek

We start with:

  • a conversation about your symptoms and medical history
  • a clinical examination of the mouth
  • one or more X-rays to accurately assess the position of the wisdom tooth

At complexe ligging (bijvoorbeeld dicht bij de zenuw of onder de kaakrand) adviseren we een CBCT-scan (3D-opname) te maken. Daarmee kunnen we de exacte ligging in drie dimensies zien en de behandeling veiliger en voorspelbaarder plannen.

2. Verdoving

Before we begin, you will receive a local anaesthetic. This makes the procedure itself painless; you may feel pressure or movement, but no sharp pain.

We always check that the anaesthetic has fully taken effect before we start.

3. Operatieve verwijdering

  • we make a small opening in the gum
  • if necessary, we remove a small amount of bone around the tooth
  • sometimes the tooth is divided into several pieces to make removal easier
  • afterwards, we rinse the area and carefully suture the gum closed

Thanks to our many years of experience with complex extractions and wisdom teeth, we can safely perform most treatments in our practice. Only in very exceptional or medically high-risk cases do we refer patients to the oral surgeon.

4. Hechten en gaasje

After the removal:

  • we usually place a gauze pad that you need to bite on for 30–60 minutes
  • if necessary, the wound will be sutured (with dissolvable stitches)
  • you will receive clear verbal and written aftercare instructions.

5. Herstel en nazorg

After the removal of a wisdom tooth, some pain and swelling are normal. In most cases, the symptoms decrease noticeably after a few days.

Important aftercare tips:

  • Bite firmly on the gauze for the first 30–60 minutes.
  • Do not rinse, smoke, or drink alcohol on the first day.
  • Use a cold pack on the outside of the cheek (with a cloth in between).
  • Eat soft foods for the first few days and chew on the opposite side.
  • Take the prescribed or recommended pain medication according to the instructions.

Contact us immediately if:

  • the pain increases after a few days instead of improving
  • you develop heavy bleeding, fever, or a swollen, warm cheek
  • If you can barely open your mouth

Having one or several wisdom teeth removed at the same time?

Together we will discuss whether it is advisable to:

  • remove one wisdom tooth at a time, or
  • remove several wisdom teeth in a single session

That depends on:

  • your general health
  • the complexity of the extractions
  • your work or study schedule (the rest period after the procedure)

We always look for the right balance between comfort, safety, and recovery.


Costs and insurance coverage

The fees for removing a tooth in the Netherlands are set nationally by the Dutch Healthcare Authority (NZa).

The kosten voor het verwijderen van een verstandskies hangen af van de moeilijkheidsgraad van de extractie (eenvoudig of chirurgisch), het aantal verstandskiezen dat in één zitting wordt verwijderd en de benodigde diagnostiek, zoals X-rays of – in complexe gevallen – CBCT (3D imaging).

Bij een standaard extractie liggen de totale kosten doorgaans tussen ongeveer € 200 en € 500. Wanneer aanvullende diagnostiek zoals een CBCT-scan nodig is of sprake is van een complexe chirurgische behandeling, kunnen de kosten hoger uitvallen.

Vooraf maken wij altijd een duidelijke kosteninschatting op basis van uw persoonlijke situatie, zodat u precies weet waar u aan toe bent en – indien gewenst – bij uw (aanvullende) tandartsverzekering kunt navragen wat er vergoed wordt. Daarnaast helpen we u desgewenst om uw verzekering te raadplegen.

Frequently Asked Questions about Wisdom Tooth Removal

Does wisdom tooth removal hurt?
The procedure is performed under local anaesthesia, so you should not feel any sharp pain. You may experience some pressure or pushing movements. If you do feel anything, we can always provide additional anaesthesia. In the case of a severely inflamed tooth, administering the anaesthetic can be slightly more sensitive and sometimes an extra injection is needed, but we always do this step by step and in consultation with you, to make the treatment as comfortable as possible.


How long will I experience discomfort after wisdom tooth removal?
After the procedure, it is normal to have post-operative pain and swelling for a few days. Symptoms usually improve significantly after 3–5 days, although healing of the gums and bone can take several weeks. With proper pain medication, cooling, and soft foods, most people recover smoothly. If you have any doubts or if your symptoms increase, you can always contact us.


When can I return to work or school?
This depends on the complexity of the extraction and the type of work you do. After a simple removal, you can often return to work or school after 1–2 days. After a more surgical or complex extraction, it is wise to allow for 2–3 days of recovery time. If your job involves heavy physical work, you may need a bit more rest; we will discuss this together during your appointment.


What can I eat and drink after wisdom tooth removal?
On the day of the procedure, we recommend:

  • avoiding hot food or drinks (to prevent bleeding)
  • soft foods such as yoghurt, lukewarm soup, mashed potatoes
  • chewing on the other side of your mouth

Avoid hard, sharp, or crumbly foods (e.g. chips, crusty bread, nuts) during the first few days, as these can get stuck in the wound. We also strongly advise against smoking and alcohol, as they can interfere with healing, for at least 48 hours after the procedure.
