When is a tooth extraction necessary?

We always first try to preserve teeth. Sometimes, however, extraction is the best or only option. An extraction may be necessary in cases including:

• A deep cavity that can no longer be restored properly
• A tooth that has broken off below the gumline
• A severe root tip infection that is no longer predictably treatable with a root canal
• A tooth that has become loose due to advanced gum disease
• A tooth that needs to be removed for for orthodontics (teeth alignment)

During the examination, we always discuss whether alternatives are possible (such as a root canal treatment) and which long-term solution is best for your teeth.

Does a tooth extraction hurt?

Many patients are surprised by how comfortable tooth extraction actually is:

• We use effective local anaesthesia
• We always check that the tooth really stunned is fully numb before starting
• You may feel pressure or movement, but not sharp pain

In the Netherlands, anaesthesia is included in the official fees for surgical procedures

After the extraction, it is normal for the area to feel sensitive for a few days. With proper pain medication and aftercare, these symptoms usually improve quickly.

How does an extraction work at 3D Tandarts?

1. Onderzoek en röntgenfoto

We start with:

• a clinical examination of the tooth
• one or more X-rays to assess the roots, any inflammation, and the surrounding structures

At A complex situations (bijvoorbeeld diep gebroken wortels of risico op zenuwschade) kunnen we een a CBCT scan (3D imaging) onwards to create an even more precise treatment plan.

2. Verdoving

We carefully numb the area around the tooth with local anaesthesia. We take the time to make sure you feel comfortable, and we only begin the procedure once you confirm that the anaesthesia is working properly.

3. Eenvoudig of moeizaam trekken

Depending on the situation, we generally distinguish between two types of extractions:

a) Simple extraction (H11)
If the tooth can still be gripped reasonably well:

• we gently loosen the tooth using an elevator
• we then remove it with forceps
• in most cases, the procedure is quick and predictable

b) Complex or surgical extraction (H35)
a complex or surgical extraction (H35): used when a tooth is fractured below the gum line, firmly ankylosed, or when the roots require surgical access:

• We open a small flap in the gum.
• We can remove a small amount of bone around the root.
• The tooth may sometimes be divided into several pieces to remove it in a controlled and safe way.

Thanks to our extensive experience with complex extractions, we can perform most of these treatments safely in our practice.

4. Wondverzorging en hechten

After the extraction:

• we place a gauze pad that you bite on firmly for 30–60 minutes to stop the bleeding
• we suture the wound with dissolvable stitches
• you will receive clear verbal and written aftercare instructions.

5. Herstel en nazorg

Good aftercare is important to prevent complications and to support faster healing.

immediately after the treatment;

• Bite firmly on the gauze for 30–60 minutes
• Do not rinse, smoke, or drink alcohol on the first day
• Use a cold pack on the cheek (with a cloth in between) to reduce swelling
• Eat soft foods and chew on the opposite side

Pain relief: We often recommend a combination or alternation of paracetamol and—if suitable for your situation—an NSAID (e.g., ibuprofen). We will discuss this with you beforehand and take your medical history into account.

When should you contact us?

• If the pain increases after a few days instead of improving
• If you have heavy bleeding that does not stop
• If you develop a fever or a significant swelling of the cheek or jaw
• If you can barely open your mouth

What comes after an extraction? (Replacement options)

Frequently Asked Questions about Tooth Extractions

Does a tooth extraction hurt?

De behandeling vindt plaats onder lokale verdoving, waardoor u geen scherpe pijn zou moeten voelen. U kunt wel druk of trekkende en wrikkende sensaties ervaren tijdens de behandeling.

Als u toch iets voelt, kunnen we altijd extra verdoven. Bij een ontstoken tand of kies kan de verdoving soms iets minder goed werken, waardoor een aanvullende verdoving nodig kan zijn. Dit gebeurt altijd rustig en in overleg, zodat de behandeling zo comfortabel mogelijk verloopt.

How long will I experience discomfort after a tooth extraction?

Na een extractie is het normaal dat u enkele dagen napijn en lichte zwelling ervaart. Meestal nemen de klachten binnen 3 tot 5 dagen duidelijk af. De volledige genezing van het tandvlees en het onderliggende bot duurt echter enkele weken.

Met goede pijnstilling, koelen en zachte voeding verloopt het herstel doorgaans voorspoedig. Als de pijn na enkele dagen juist toeneemt in plaats van afneemt, is het belangrijk om contact met ons op te nemen.

Wat mag ik eten en drinken na de behandeling?

Op de dag van de behandeling adviseren wij te wachten tot de verdoving grotendeels is uitgewerkt, om te voorkomen dat u per ongeluk op uw lip of wang bijt.

Kies daarna voor zachte en lauwe voeding zoals yoghurt, soep of puree, en kauw bij voorkeur aan de andere kant van de mond. Vermijd hete dranken, alcohol en roken, omdat dit het risico op nabloeding verhoogt en de genezing kan vertragen.

In de dagen erna kunt u uw voeding geleidelijk uitbreiden, maar wees in het begin voorzichtig met harde of scherpe producten zoals noten, chips en stokbrood.

When can I return to work or sport?

Na een eenvoudige extractie kunt u meestal na 1 tot 2 dagen uw dagelijkse activiteiten weer hervatten.

Bij een chirurgische of complexere extractie, of wanneer u fysiek zwaar werk verricht, is het verstandig om rekening te houden met 2 tot 3 dagen rust. Intensief sporten, zwaar tillen en contactsport worden in de eerste dagen afgeraden om het risico op nabloeding te beperken.

Wat kost het trekken van een tand of kies en wordt het vergoed?

De tarieven voor het trekken van tanden en kiezen worden in Nederland vastgesteld door de NZa (Nederlandse Zorgautoriteit). De kosten zijn afhankelijk van de complexiteit van de behandeling, het aantal te verwijderen elementen en de benodigde diagnostiek, zoals röntgenfoto’s of aanvullende beeldvorming.

Bij een eenvoudige extractie liggen de kosten, inclusief consult en röntgenfoto’s, meestal tussen ongeveer € 100 en € 250. Bij chirurgische of complexere behandelingen kunnen de kosten hoger uitvallen.

Wanneer een CBCT-scan (3D-röntgen) nodig is voor een nauwkeurige diagnostiek en behandelplanning, bedragen de kosten hiervoor € 255,06.

Wij maken altijd vooraf een duidelijke kosteninschatting, zodat u precies weet waar u aan toe bent en dit indien gewenst kunt afstemmen met uw (aanvullende) tandartsverzekering.

Moet een getrokken tand of kies altijd vervangen worden?

Dat hangt af van de locatie in de mond en uw gebitssituatie. Een ontbrekende tand of kies kan invloed hebben op de kauwfunctie, de stand van andere tanden en kiezen en in sommige gevallen ook op de esthetiek.

Samen bekijken we of vervanging nodig en wenselijk is, en welke oplossing het beste bij u past, zoals een implantaat met kroon, een brug of een (gedeeltelijke) prothese.

Wanneer heb ik een CBCT nodig?

A CBCT-scan (3D-röntgen) is niet standaard nodig bij elke extractie, maar wordt vooral ingezet bij complexe situaties. Bijvoorbeeld bij verstandskiezen die dicht bij de zenuw liggen, onduidelijke wortelposities, een vermoedelijke nauwe relatie met belangrijke anatomische structuren of wanneer een gewone röntgenfoto onvoldoende informatie geeft.

Met CBCT kunnen we de anatomie zeer nauwkeurig beoordelen, waardoor de behandeling beter kan worden gepland en veiliger en voorspelbaarder kan worden uitgevoerd.

Wanneer moet ik direct contact opnemen na een extractie?

Neem direct contact met ons op bij een aanhoudende of hevige nabloeding, toenemende pijn of zwelling na enkele dagen, koorts, slikklachten of wanneer u uw mond steeds minder goed kunt openen.

In deze situaties beoordelen wij de klachten en plannen we indien nodig een spoedcontrole in.
