When is a tooth extraction necessary?
We always first try to preserve teeth. Sometimes, however, extraction is the best or only option. An extraction may be necessary in cases including:
- A deep cavity that can no longer be restored properly
- A tooth that has broken off below the gumline
- A severe root tip infection that is no longer predictably treatable with a root canal
- A tooth that has become loose due to advanced gum disease
- A tooth that needs to be removed for for orthodontics (teeth alignment)
During the examination, we always discuss whether alternatives are possible (such as a root canal treatment) and which long-term solution is best for your teeth.
Does a tooth extraction hurt?
Many patients are surprised by how comfortable tooth extraction actually is:
- We use effective local anaesthesia
- We always check that the tooth really stunned is fully numb before starting
- You may feel pressure or movement, but not sharp pain
In the Netherlands, anaesthesia is included in the official fees for surgical procedures
After the extraction, it is normal for the area to feel sensitive for a few days. With proper pain medication and aftercare, these symptoms usually improve quickly.
How does an extraction work at 3D Tandarts?

– Examination and X-ray
We start with:
- a clinical examination of the tooth
- one or more X-rays to assess the roots, any inflammation, and the surrounding structures
In complex situations (e.g. deeply broken roots or risk of nerve damage), we may recommend a CBCT (3D imaging) from 2026 for an even more precise treatment plan.
–Anaesthesia
We carefully numb the area around the tooth with local anaesthesia. We take the time to make sure you feel comfortable, and we only begin the procedure once you confirm that the anaesthesia is working properly.
- Simple or complex extraction
Depending on the situation, we generally distinguish between two types of extractions:
a) Simple extraction (H11)
If the tooth can still be gripped reasonably well:
- we gently loosen the tooth using an elevator
- we then remove it with forceps
- in most cases, the procedure is quick and predictable
b) Complex or surgical extraction (H35)
a complex or surgical extraction (H35): used when a tooth is fractured below the gum line, firmly ankylosed, or when the roots require surgical access:
- We open a small flap in the gum.
- We can remove a small amount of bone around the root.
- The tooth may sometimes be divided into several pieces to remove it in a controlled and safe way.
Thanks to our extensive experience with complex extractions, we can perform most of these treatments safely in our practice.
-Wound care and suturing
After the extraction:
- we place a gauze pad that you bite on firmly for 30–60 minutes to stop the bleeding
- we suture the wound with dissolvable stitches
- you will receive clear verbal and written aftercare instructions.
Recovery and aftercare
Good aftercare is important to prevent complications and to support faster healing.
immediately after the treatment;
- Bite firmly on the gauze for 30–60 minutes
- Do not rinse, smoke, or drink alcohol on the first day
- Use a cold pack on the cheek (with a cloth in between) to reduce swelling
- Eat soft foods and chew on the opposite side
Pain relief: We often recommend a combination or alternation of paracetamol and—if suitable for your situation—an NSAID (e.g., ibuprofen). We will discuss this with you beforehand and take your medical history into account.
When should you contact us?
- If the pain increases after a few days instead of improving
- If you have heavy bleeding that does not stop
- If you develop a fever or a significant swelling of the cheek or jaw
- If you can barely open your mouth

What comes after an extraction? (Replacement options)
Having a tooth removed is often just the first step. Afterward, we look at how we can best restore chewing function and aesthetics.
Possible options:
- Implant with crown (if there is enough bone and a healthy surrounding area)
- Bridge supported by the neighbouring teeth
- Partial denture (frame or acrylic) for multiple missing teeth

Costs and insurance coverage
The fees for removing a tooth in the Netherlands are set nationally by the Dutch Healthcare Authority (NZa).
As a guideline, including the necessary X-rays, the total cost is usually between €100 and €250.

Frequently Asked Questions about Tooth Extractions
Does a tooth extraction hurt?
The treatment itself takes place under local anaesthetic, so you should not feel any sharp pain. You may, however, experience some pressure or prying.
If you do feel something, we can always additional annealing. If the tooth is highly inflamed, the anaesthetic may be more sensitive and it is sometimes necessary to give an additional anaesthetic injection. We always do this calmly, step by step and in consultation, so that you experience the treatment as comfortably as possible.
How long will I experience discomfort after a tooth extraction?
After an extraction, it is normal to have post-operative pain and mild swelling for a few days. Symptoms usually improve noticeably after 3–5 days, although full healing of the bone and gums can take several weeks.
With proper pain medication, cooling, and soft foods, most people recover smoothly. If your symptoms worsen instead of improve, please contact us.
What can I eat and drink after the procedure?
On the day of the treatment, we recommend:
- to wait until the anaesthesia has mostly worn off first (to avoid biting your lip or cheek);
- to choose soft, lukewarm foods (e.g. yoghurt, soup, mashed potatoes);
- to chew on the other side of your mouth;
- to avoid hot drinks, alcohol, and preferably not to smoke (as this increases the risk of bleeding and slows down healing).
In the following days, you can gradually expand your diet, but avoid hard or sharp foods such as nuts, chips, and crusty bread in the beginning.
When can I return to work or sport?
After a simple extraction, you can usually after 1-2 days back to work or school.
For more difficult extraction or if you do physically demanding work, it is wise to consider 2-3 days rest. Intensive sports, heavy lifting and contact sports are best postponed for a few days to prevent post-operative bleeding. If necessary, we will discuss this during your appointment.
What does a tooth extraction cost and is it covered by insurance?
The costs depend on:
- whether it is a simple or a complex/surgical extraction,
- whether one or multiple teeth are extracted during the same appointment,
- and which x-rays are required.
The rates are determined by the NZa. For adults, reimbursement falls under the additional dental insurance; without supplementary insurance, the costs are at your own expense.
We are happy to provide a cost estimate in advance so that you can check this with your insurance provider if needed. Including consultation and x-rays, total cost are generally between 100 and 250 Euro.
Does a removed tooth always need to be replaced?
That depends on the position in the mouth and on your bite. A missing tooth can:
- affect your chewing function;
- look esthetically unappealing (especially in the front);
- cause other teeth to shift.
Together we will assess whether replacement is advisable and which options are suitable for you, such as a dental implant with crown, a bridge, or a (partial) denture.
When should I contact you immediately after an extraction?
Please contact us as soon as possible if you:
- have heavy bleeding that does not stop;
- develop increasing pain, swelling, or fever after a few days;
- develop a swollen, warm cheek or difficulty swallowing;
- can barely open your mouth.
In that case, we will assess the situation and schedule an emergency check-up if needed.
