Depending on your situation, the plan could include, for example:

How much does it cost?
The Smile Consult is declared at 3D Tandarts as an occasional consultation (C003) according to NZa rates.
The rate for C003 is € 28,51.
Invoicing is done via Infomedics, who will send the invoice on our behalf and charge any co-payment to you.
What should you bring?

- A valid ID (required for registration).
