What is a Root Canal Treatment (RCT)?
Root canal treatment — also called endodontic therapy — is performed when the soft tissue inside a tooth (the dental pulp) becomes infected or damaged due to deep decay, trauma, or fractures. Without treatment, the infection can spread, causing pain or abscess.
This can be caused by, among other things:
- Deep cavities (tooth decay)
- A leaking old filling
- A crack or fracture in the tooth
- A blow or an accident
Inflamed pulp does not heal on its own. If nothing is done, the infection can spread to the bone around the root, causing more pain, swelling, and sometimes even an abscess. With a root canal treatment, we thoroughly clean the inside of the tooth so the infection disappears and the tooth can be preserved.
Does a root canal treatment hurt?
Most patients are surprised at how comfortable a root canal treatment actually is
- We treat almost always under local anaesthesia; the tooth and surrounding tissues are carefully numbed.
- We take the time to let the anaesthesia take effect properly.
- During the treatment, we can always pause if you need it.
The purpose of the treatment is actually to relieve your pain, not to cause additional pain. After the treatment, the tooth may remain slightly sensitive for a few days, but this is usually well managed with regular painkillers.
How does a root canal treatment work at 3D Tandarts?
– Preliminary examination and diagnosis
We always start with a clinical examination and an X-ray. This allows us to assess:
- How deep the infection is
- How many roots and canals the tooth has
- Whether there is already an infection visible around the root tip
In complex cases or when in doubt, we may recommend additional imaging; from 2026 onwards, we will expand this with a 3D CBCT scan for even more precise diagnosis and planning in challenging endodontic treatments.
– Anaesthesia and rubber dam
Local anesthesia is administered. The tooth is isolated using a rubber dam to prevent contamination, providing:
- A clean and dry working field
- Additional safety (no instruments or rinsing fluid in the mouth)
- Comfort and hygiene during treatment
– Working under magnification (microscope)
We work under magnification (microscope/magnifying loupes).. This allows us to:
- Locate fine root canals more easily
- Follow bends and branches more accurately
- Remove old root fillings more effectively during retreatment
The better we can visualise the canals, the greater the chance of a long-lasting result.
– Cleaning and disinfecting the canals
An access cavity is prepared through the crown, allowing entry into the root canals.
Infected pulp is removed, and the canals are cleaned, shaped and chemically disinfected using antiseptic irrigants.
- We remove the infected pulp
- We clean the canals along their full length
- The canals are chemically disinfected and, in some cases, also treated with the assistance of a dental laser.
In more complex cases, we additionally use a diode laser to further disinfect the canals. The laser helps reduce bacteria deeper within the canal walls — an extra step for complex or repeatedly infected teeth.
– Filling the root canals
Once the canals are clean and dry, they are three-dimensionally padded with a special filling material and a root cement. This prevents re-invasion of bacteria as much as possible.
– Temporary or final restoration
After the root canal treatment:
- We usually first place a temporary filling
- Depending on the remaining tooth structure, a permanent restoration (filling or crown) is placed to protect and strengthen the tooth.
For molars, we often recommend a crown to prevent future fractures.
One or multiple appointments?
Depending on the situation, one or several sessions may be required:
- Simple cases with minimal inflammation can often be completed in a single appointment.
- Complex root canals, retreatments, or severe infections are usually performed in two or more appointments for safety reasons
During the first consultation, we discuss:
- How many appointments are likely needed
- How long each appointment will take
- A clear cost estimate according to the national NZa dental fee schedule.
How much does a root canal treatment cost?
The cost of a root canal treatment depends on several factors, such as which tooth is being treated, the number of root canals, the complexity of the case, and whether one or multiple appointments are required. The fees are based on various NZa codes (examination, anaesthesia, treatment per canal, x-rays, etc.).
As an indication, the cost of a root canal treatment for adults, excluding the final filling or crown, is usually between € 400 and € 1.000. We always provide a clear cost estimate in advance, so you know what to expect and can check this with your supplementary dental insurance if needed.
For children/young patients:
For all patients under 18 years of age who are insured in the Netherlands (with a BSN and basic health insurance), a root canal treatment is generally fully covered by the basic insurance. You do not pay any personal contribution, and no deductible applies to this youth dental care.

When should you call us immediately?
Contact us promptly if you experience one or more of the following symptoms:
- Severe, throbbing toothache
- Pain when biting or chewing
- Persistent sensitivity to heat or cold
- Swelling of the gums or cheek
- A pimple-like bump (fistula) on the gum
These symptoms may indicate nerve inflammation or an infection around the root tip. The sooner we can take action, the greater the chance of saving the tooth.
Frequently Asked Questions (FAQ)
Does a root canal treatment hurt?
No, the procedure is performed under local anaesthesia. You may feel some pressure, but not sharp pain.
If the tooth is severely inflamed, the anaesthetic may feel slightly more sensitive or may need to be topped up, but we always do this calmly and in consultation with you to ensure maximum comfort.
After the treatment, the tooth may remain sensitive for a few days; this is usually easy to manage with standard pain medication.
How long does the treatment take?
Depending on the number of roots and the complexity of the case, a root canal treatment usually takes between 60 and 90 minutes per toothFor complex cases or retreatments, we may schedule more than one appointment.
Will I have symptoms after the treatment?
Mild discomfort or sensitivity when biting is normal during the first few days and usually improves quickly.
