What causes tooth pain?
Toothache can come on suddenly and is often one of the most uncomfortable types of pain a person can experience. It may start as a dull, lingering ache that gradually gets worse, or as a sharp, sudden pain that appears out of nowhere. In either case, it can make eating, drinking, and even sleeping difficult – and it often brings a sense of worry with it.
Many people initially try to relieve the pain with painkillers, and while this can provide temporary relief, it only masks the symptoms. Painkillers don’t address the underlying cause. Toothache is almost always the result of an inflammation or irritation that can worsen if left untreated.

Pulpitis - Inflammation of the tooth nerve
Symptoms: sharp pain with hot or cold and pain at night. Causes: deep cavities, leaking fillings, traumas. Treatment: root canal treatment or tooth extraction.
One of the most frequent causes of toothache is pulpitis: an inflammation of the dental nerve (pulp) located in the innermost part of the tooth.
Pulp tissue contains nerves and blood vessels that nourish the tooth from within and make it sensitive to temperature and pressure. When this pulp becomes irritated or inflamed, it can cause pain—often sharp, throbbing, or aching—that may increase in intensity over time.
From reversible to irreversible inflammation
In the early stage, the inflammation may still be reversible : the pulp is irritated but can recover once the underlying cause—such as tooth decay or a leaking filling—is removed.
At this stage, the pain is usually short-lived and occurs in response to cold or sweet stimuli.
However, when the irritation persists, pressure inside the tooth increases, blood flow decreases, and the tissue becomes permanently damaged—this is known as irreversible pulpitis.
Without treatment, the nerve may eventually die (necrosis). The inflammation can then spread beyond the root tip into the surrounding bone, causing an apical infectionthis may lead to throbbing pain, swelling, or even an abscess.
If left untreated, an infection in the jaw can also affect overall health, as bacteria may enter the bloodstream and place a prolonged burden on the immune system.
🔹 Causes of pulp inflammation
- Deep Tooth Decay (Caries)
- Leaking or Aged Fillings
- Tooth trauma (such as a fall or heavy bite)
- Repeated Dental Procedures on the same tooth
- Untreated Gum Infections that spread to the root area
🔹 Treatment
The treatment depends on the stage of the inflammation:
- In case of reversible pulpitis:
The tooth can usually be preserved with a temporary filling or a direct restoration after the affected tissue has been removed.
After a few days, the situation is re-evaluated. If the pain has disappeared and the nerve has recovered, a permanent filling or crown will be placed to provide lasting protection for the tooth. - In case of irreversible pulpitis:
The only way to stop the infection and save the tooth is through a root canal treatment (endodontic therapy). During this procedure, the inflamed or dead pulp is removed, and the canals are cleaned and sealed to prevent reinfection.
In some cases, when the tissue and root structure are too severely damaged, an extraction (tooth removal) is necessary.
After the extraction, it may later be possible to replace the missing tooth with an implant or a bridge.

The earlier pulpitis is recognized and treated, the greater the chance of saving the tooth and preventing complications.
Timely intervention eliminates the source of infection, prevents it from spreading to the jawbone or other teeth, and also protects your overall health.
Do you need a root canal treatment?
At 3D Tandarts, we work with magnification to perform the treatment as precisely as possible as precisely as possible

For stubborn infections around the root tip, we also use a diode laser for extra-deep disinfection of the root canal system.
Periodontitis - Gum disease
Symptoms: bleeding gums, loose teeth, bad breath. Causes: plaque and tartar under the gums. Treatment: professional cleaning, laser therapy, periodontal treatment.
Sometimes toothache does not come from the nerve, but from the supporting tissues around the tooth: the gums and bone holding the tooth in place.
When bacteria accumulate between the tooth and the gum, it causes inflammation of the gums (gingivitis).
If this is not treated in time, the inflammation can spread deeper and affect the jawbone — this stage is called periodontitis.
In case of periodontitis the gums detach from the tooth forming pockets where bacteria and tartar build up.
As a result, the tooth loses its support and may eventually become loose or even fall out.
Sometimes abscesses may develop — sudden, painful swellings filled with pus — which require immediate treatment.
🔹 Causes and risk factors
Periodontitis develops due to a combination of bacterial buildup and individual risk factors that weaken gum defences or delay healing.
Most important causes
- Plaque and tartar buildup under the gums.
- Insufficient or irregular oral hygiene
- Food debris or poorly fitting fillings and crowns where bacteria can accumulate
Risk factors that can worsen the situation
- Smoking: reduces blood supply to the gums and slows healing
- Stress: affects the immune system and increases susceptibility to inflammation
- Diabetes: increases vulnerability to infections and delays recovery
- Genetic predisposition: some people have a hereditary susceptibility to periodontal problems
- Hormonal changes: for example during pregnancy or menopause
- Poor nutrition or vitamin deficiency
It is essential to recognize and minimize the risk factor as much as possible.
Quitting smoking, keeping diabetes under control, and attending regular dental check-ups are important steps to prevent or stabilize periodontitis.
Consistent and proper mouth hygiene — brushing twice a day with a soft toothbrush and cleaning between the teeth daily — is the foundation for long-term success.

🔹 Treatment of periodontitis
1️⃣ Acute phase treatment
In the first phase, it is important to quickly control the inflammation and reduce pain.
This is achieved through professional, thorough cleaning of the gum pockets - treatment also known as scaling and root planing - where tartar and bacteria are removed from the gums.
This treatment is performed manually with fine instruments and can often provide noticeable relief after just one session: the gums become calmer, and pressure and discomfort decrease.
2️⃣ Support with Diode Laser
In case of acute inflammation the use of a diode laser can offer more advantages.
The laser helps reduce bacteria deep within the gum pockets and promotes tissue healing, often resulting in less post-treatment discomfort and faster recovery.
3️⃣ Advanced cases treatment
When the bone has been severely damaged, it may sometimes be necessary to perform a surgical treatment in extreme cases, to remove the tooth in order to prevent to prevent further spread of the infection.
After extraction, we always discuss options to restore the dentition, such as implants or bridges.
4️⃣ Aftercare and maintenance
Treating periodontitis is not just a one-time procedure.
To prevent recurrence, a regular maintenance program is essential:
- Professional cleanings every 3 to 6 months
- Good mouth hygiene at home (tooth brushing and interdental cleaning)
- Regular check-ups at the dentist of the pocket depth

Erupting or inflamed wisdom teeth
Symptoms: pain in the back of the jaws, swelling, reduced mouth opening. Causes: partially erupted tooth or pressure from the impacted root. Treatment: cleaning, medications, or tooth removal
The wisdom teeth, or third molars, are the last teeth to come out - mostly between the ages of 18 and 25 years.
For many people, they emerge without problems and can play a full role in chewing.
However, it is common for there to be insufficient space in the jaw, or for the tooth to grow at an angle and only partially erupt.
In those cases, the condition is called disodontiasis: a difficult or incomplete eruption of the wisdom tooth.
This situation can occur in young people, but also later in life, and is often accompanied by pain, swelling, or inflammation of the gum at the back of the mouth.
The pain can radiate to the ear or jaw and sometimes make opening the mouth difficult (trismus).
When the inflammation spreads, even swallowing or chewing can become painful.
🔹 When Wisdom Teeth Cause Problems
Wisdom teeth can cause various problems:
- Pain and swelling in the back of the jaw
- Inflammation of the gum (pericoronitis)
- Areas that are difficult to clean, where food debris and bacteria can accumulate
- Pressure on adjacent teeth, which can lead to shifting or damage to the second molar
- Cyst formation or, rarely, infection of the jawbone
Even fully erupted wisdom teeth can cause problems later, especially if they are difficult to keep clean or if the surrounding gum remains irritated.
🔹 Preventive Removal — Sometimes Better to Act Early
Although not every wisdom tooth needs to be removed, in some cases it can be wise to do so preventively — for example, when:
- The tooth clearly does not have enough space to fully erupt
- The position makes the cleaning not possible, increasing the risk of decay or infection
- There have already been repeated infections around the same tooth
- There is a risk of pressure or damage to the neighbouring teeth
Early removal, even before the roots are fully formed is usually easier and heals faster.
For this reason, we sometimes recommend having wisdom teeth evaluated with an X-ray or CBCT scan around the age of 17-18 .
🔹 Treatment and removal
In most of the cases the extraction of the wisdom teeth is performed safely and easily in our dental practice 3D Tandarts.

Dental Trauma
Symptoms: tenderness to touch, discoloration, broken tooth, loose tooth. Causes: fall, sport accidents, hard bite. Treatment: splint, filling, reparation with composite, indirect restoration, root canal treatment.
A dental trauma often occurs suddenly — from a fall, a sports injury, or simply biting on something hard.
The damage can range from a small chipped edge to a completely loose or knocked-out tooth.
Even in seemingly minor injuries, it’s important to have the tooth examined as soon as possible, since the nerve or jawbone may be damaged without visible signs.
A quick diagnosis can make the difference between saving the tooth or having to remove it.
🔹 Types of Dental Trauma
- Broken tooth (fracture): a piece of the tooth breaks off, often causing sensitivity to cold or pressure.
- Loose or displaced tooth: the tooth is still in place but moves or has shifted in the dental arch.
- Intrusion or extrusion: the tooth has been pushed deeper into the bone or pulled outward.
- Avulsion (completely knock-out tooth): the tooth has come entirely out of the mouth — this is a dental emergency.
In children, baby teeth can also be affected; in such cases, the treatment differs, as the permanent teeth have not yet erupted.
🔹 What to do in case of accident
- Stay calm and act quickly.
- If the tooth has been knocked out :
- Do not touch the root, but hold the tooth by the crown.
- Rinse it briefly with milk or saline solution, not with water.
- If possible, gently place the tooth back into its socket.
- If that’s not possible, store it in milk or under the tongue and call your dentist immediately.
Every minute counts — the sooner the tooth is reimplanted or treated, the greater the chance of saving it.
🔹 Treatment of dental trauma
Treatment depends on the severity of the injury:
1️⃣ Small fractures
Small broken pieces can often be repaired with composite (white filling) that restores the natural shape
For larger fractures, a crown or buildup may be needed to provide additional protection.
2️⃣ Damage to the nerve (pulp)
When the nerve is exposed or becomes inflamed after trauma, a root canal treatment is often required to save the tooth and prevent further infection.
Prompt treatment increases the chance of full recovery of the tooth.
3️⃣ Loose or Displaced Teeth
The tooth is carefully repositioned in its own place and then temporarily splinted (attached to the neighboring teeth) to provide stability during healing.
After a few weeks, a follow-up examination is performed to check whether the tooth root remains vital.
4️⃣ Knocked-Out Teeth (Avulsion)
If the tooth is reimplanted in time, it can often be preserved, although a root canal treatment may be needed later.
In cases of permanent loss, we discuss replacement options such as implants or bridges.
🔹 Aftercare and Follow-Up
After a trauma follow-up is essential, even if the tooth feels normal again.
Inflammation or discoloration may still appear weeks or months later.
At 3D Tandarts, we carefully monitor such cases with regular check-ups and X-rays to ensure that the tooth remains healthy.
Periapical infection
Symptoms: throbbing pain, swelling, sensitivity while chewing. Causes: non-treated pulpitis or infection around the root tip. Treatment: root canal treatment, root canal retreatment, apical resection, or eventually extraction.
When pulpitis is not treated in time, the inflammation can spread beyond the tooth root into the surrounding bone. This condition is known as an apical infection or periapical infection.
The infection causes a buildup of bacteria and inflammatory fluid at the tip of the root, leading to pressure and pain . It often feels like a throbbing, constant pain that worsens when biting or chewing.
Sometimes the infection can develop silently: the pain decreases temporarily, but the inflammation remains and may later spread or flare up again.
An untreated periapical infection poses a risk not only to the tooth itself but also to the surrounding bone and even to overall health.
🔹 Treatment of the periapical infection
The goal of the treatment is always to eliminate the bacterial infection, allow the tissue to heal and - if possible - to save the tooth.
1️⃣ Root canal treatment (or retreatment)
In most cases, a periapical infection can be resolved through a root canal treatment or a root canal retreatment of a previously treated canal.
During this procedure, the infection inside the root is removed, the canals are thoroughly cleaned, and then sealed again to keep bacteria out.
Once the infection is under control, the body can gradually heal the area, allowing the bone to recover and the tooth to be preserved.
2️⃣ Apicoectomy (apical surgery)
If the infection persists despite a proper root canal treatment, a surgical procedure can be performed, this is called an apicoectomy.
Through a small opening in the gum, the inflamed tissue around the root tip is removed together with a tiny portion of the root itself.
The end of the root canal is then sealed again with a special filling material.
This is a precise and predictable procedure that often allows the natural tooth to be saved.
3️⃣ Extraction (tooth removal)
In the most severe cases — for example, when the tooth is too structurally damaged or there has been significant bone loss — it may be necessary to remove the tooth.
After extraction, we will discuss suitable replacement options, such as an implant or a bridge, to fully restore both function and aesthetics.
🔹 Why quick action is required
A periapical infection, just like pulpitis, can also affect the entire body.
Bacteria from the infection can spread through the bloodstream and place a burden on the immune system.
By intervening in time, we can remove the source of infection, often preserve the tooth, and prevent future recurrence.
Teeth grinding? Jaw joint complaints?
Symptoms: pain when chewing, clicking or popping of the joint, headache. Causes: stress, grinding (bruxism), misaligned bite. Treatment: bite splint, physiotherapy, laser therapy, bite adjustment.
Not all toothache comes from the teeth or gums — sometimes the cause lies in the jaw joint (TMJ) or the chewing muscles.
This joint connects the lower jaw to the skull and is constantly in motion during talking, chewing, and swallowing.
When this delicate system becomes unbalanced, it is called temporomandibular dysfunction (TMD).
This can lead to pain, stiffness, and even headaches or earaches.
🔹 Symptoms of jaw joint problems
- Pain or pressure around the jaw or ear
- Clicking or popping sounds when opening the mouth
- Limited mouth opening or a “locked” jaw
- Muscle tension in the neck and temples
- Headaches, neck pain, or pain radiating to the shoulders
Sometimes it feels like tooth pain, even though the teeth themselves are perfectly healthy.
These symptoms can be temporary, but they can also become chronic if not addressed.

- Overuse due to teeth grinding or clenching (bruxism)
- Stress or tension expressed in the jaw muscles
- Misaligned bite or uneven jaw forces
- Trauma or sudden jaw movement
- Joint wear or inflammation
The combination of muscle fatigue and inflammation causes pain and limits jaw mobility.
